The Death Certificate also states that he died on August 5, 1928, while his tombstone states that he died on August 25, 1928. I have used the date listed in the Death Certificate. His Death Certificate also appears to state this his name is Glin Dale, although his tombstone states that his name was Glen Dale. His Death Certificate is No. 19390.
